CSB Foundation Guidelines
In order to assure fairness, consistency and as much objectivity as possible in the grant allocation process, all grant requests will be accepted on a common basis. The Foundation Board will not normally consider a grant request unless it includes in writing:
- Completed Application Form.
- Proof that the organization is tax exempt under Section 501{c}(3) of the Tax Reform Act of 1969, with the exception of municipalities and their subdivisions.
- Amount requested
- One to three page memo describing the applicant organization and the program for which it seeks funding.
- Copy of the organization’s most recent annual report and current financial statements (or other documents which provide essentially a financial picture of the organization). Administrative costs, as a percentage of the budget, should be provided.
- Contact person’s name and daytime phone number.
- General description of the organization’s current and future financial resources.

Grant applications will be received in May and grants will be awarded by June 30th of each year.
